We aim to contact you to confirm we have received the application, within 3 to 5 working days of receiving the form.

 

At this time, the receptionist will send you further forms: the Case History form, Consent to Assess form, consent to disclose form, coordination questionnaire (year 7 and above) and FAQ. These forms are necessary before client's files can be reviewed by the assessors. and added to the waitlist.

2. Appointment time

When an appointment time becomes available, the receptionist will call you to arrange an assessment time. Our waitlist is currently 3 months, but this does depend on availability of the assessors and the type of assessment required.
